Behaviour Change Clinician (Telehealth)

Contract Position | Work from Home | Flexible Hours - approximately 3-5 hours per week

Dr Kate Brunsdon is establishing a premium preventive health and healthy ageing practice focused on helping people optimise their health today while preserving strength, function and independence for the future.

Following a comprehensive medical assessment, every patient receives a personalised Health Roadmap identifying their key health priorities and evidence-based recommendations. We are seeking an Accredited Mental Health Social Worker to work alongside Dr Kate by supporting patients to successfully implement these recommendations through structured behaviour change consultations.

This is a unique opportunity to become part of an innovative multidisciplinary model of care that combines medical assessment, allied health and behavioural support.

About the role

Patients referred to you will already have:

  • A comprehensive medical assessment
  • Individualised risk stratification
  • A personalised Health Roadmap
  • Clearly prioritised health goals
  • Exercise, nutrition and medical recommendations where appropriate

Your role is to help patients successfully implement these recommendations through evidence-based behaviour change strategies.

Consultations will be delivered via telehealth and may focus on:

  • Building sustainable habits
  • Increasing motivation and confidence
  • Breaking unhelpful behaviour patterns
  • Accountability and ongoing support
  • Problem-solving barriers to change
  • Helping patients translate intention into action

Common areas of focus may include:

  • Alcohol reduction
  • Emotional or binge eating
  • Weight management behaviours
  • Exercise adherence
  • Sleep routines
  • Stress management
  • ADHD-related executive functioning challenges
  • Building healthier daily routines
  • Maintaining long-term lifestyle change

Essential Requirements

  • Accredited Mental Health Social Worker (AASW)
  • Current professional indemnity insurance
  • Experience delivering telehealth consultations

Highly Desirable Experience

Applicants with experience in one or more of the following will be highly regarded:

  • Motivational Interviewing
  • Acceptance and Commitment Therapy (ACT)
  • Behaviour change counselling
  • Addiction services
  • Obesity or metabolic health
  • Lifestyle medicine
  • Chronic disease management
  • Pain management
  • Preventive health
  • Women's health

You understand that successful health change requires more than knowledge—it requires understanding motivation, habits, values, barriers and behaviour.

You are collaborative, practical and evidence-based, and enjoy working closely with other healthcare professionals to help patients achieve long-term health outcomes.
