Transplant Australia is a national charity dedicated to supporting transplant recipients, donor families, and those on the transplant journey. We deliver programs that promote physical activity, wellbeing, connection, and long-term support beyond the clinical setting.
The Social Worker (WA Families) role is a funded role to support the delivery of a pilot program supported by Telethon that will provide holistic, person-centred support to children and families in Western Australia navigating the transplant journey.
This role focuses on bridging the gap between hospital-based care and community-based support, ensuring families feel connected, informed, and supported at every stage. This role will also contribute to the development of Transplant Australia family recourses for ongoing support.
This is a pilot position, currently funded until December 2026, designed to test and evaluate a community-based support model for transplant families in WA.
