Job Description

Position Title:  Social Worker - Senior (GEMHITH) (Hervey Bay)

Location:  Geriatric Evaluation Management, Hospital in the Home, Integrated Care Services, Hervey Bay Hospital

Remuneration:

You will receive a competitive remuneration package value up to $78,680 p.a., comprising salary rates $64.64 - $69.56 p.h., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (HP4) (Permanent Part Time, 38 hours p.f.). (Applications will remain current for 12 months).

Your opportunity:

  • Deliver high level clinical Social Work services and professional expertise to patients / stakeholders of the GEM/GEMR HITH service. The position will provide leadership in the coordination and delivery of evidence-based Social Work services across the care continuum to ensure optimal assessment, clinical intervention, and discharge planning occurs for health service consumers as part of a broader multidisciplinary team.
  • Work collaboratively with consumers and multidisciplinary professionals to provide high level clinical care and a therapeutic environment conducive to the dignity and wellbeing of consumers and carers within Mental Health and Specialised Services. Provide high level profession specific services, consultation and support, as required, within the team and service.
