Social Work Services

The Social Work team across Monash Health is responsible for the delivery of a broad range of Social Work services working with people across the lifespan in the inpatient and outpatient setting.   

About the Role

The Social Work Renal program is looking for two Grade 2 clinicians to join the team working across Casey, Clayton, Cranbourne, Dandenong and Moorabbin. This is an interesting and dynamic role with opportunities to work in satellite dialysis, home haemodialysis, peritoneal dialysis and provide support for the acute nephrology inpatient ward.

Position 1 0.5EFT permanent (2.5 days per week or 5 days per fortnight) based at Casey and Cranbourne sites

Position 2 0.8EFT temporary to March 2022(4 days per week) based at Thomas St, Dandenong

For both positions work across the spectrum of Nephrology and all sites may be required.  Experience with chronic illness and health social work are desirable.   

This clinical role provides an opportunity to showcase your skills working with people from diverse backgrounds including completing complex psycho-social assessments and developing patient centred care plans with patients and families facing life-threatening situations and decision making. The diversity of the role requires clinical knowledge that ensures a safe, high quality Social Work service for all clients.

Deliver and drive Monash Health strategic intent to be relentless in our pursuit of excellence across Social Work and Allied Health,
Work closely with the Social Work Manager to provide streamlined services across all campuses and lead designated services/programs/portfolios,
Travel is required across Monash Health sites.
About You

You will be a dynamic, innovative and flexible clinical practitioner with a good understanding of the treatment and psycho-social issues facing people with chronic illness and their families. 

You will demonstrate skills in psycho-social assessments, problem solving, communication, decision-making and hospital discharge planning and have an understanding of the

aged care sector,
chronic health conditions,
NDIS and related services,
People at risk
What you need

You must be eligible for AASW membership including continuing professional education and supervision
Experience in providing social work services in health or community health settings.
Knowledge and experience working closely with inter-professional teams.
Current Driver’s Licence.
Current Working with Children’s Check
