Carers NSW is the peak body in NSW for carers of all ages and circumstances. We are a not-for-profit organisation connecting and empowering carers and improving their quality of life through support, referrals, capacity building and training.
As the Carer Gateway Service Provider (CGSP) in Region 4 of NSW (Central Coast, Hunter New England and North Coast) we have full time & part time contract opportunities for Carer Service Coordinators based in Coffs Harbour or Newcastle.
CARER SERVICE COORDINATORS:
These positions are responsible for liaising with providers and carers to arrange in-home respite services as part of the Carer Gateway. At times, this role will also support carers through the Carer Support Planning Process including an intake, registration and needs assessment, support planning, coordination and support monitoring to ensure carers can be holistically supported by understanding their needs and referring them to appropriate supports.
After hours on-call availability is required overnight and on weekends as needed, to arrange emergency respite for carers.
In addition, knowledge of the Central Coast, Hunter New England and North Coast (Region 4) will be highly regarded.
