Position Title: Social Worker - Senior (Integrated Care Rehabilitation Services) (Bundaberg, Hervey Bay and Maryborough)
Location: Integrated Care Rehabilitation Services, Bundaberg Hospital and Hervey Bay and Maryborough Hospitals
Remuneration:
You will receive a competitive remuneration package value up to $78,680 p.a., comprising salary rates $64.64 - $69.56 p.h., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (HP4) (Two Permanent Part Time Positions, 38 hours p.f.). (Applications will remain current for 12 months).
Your Opportunity:
To provide high level Social Work clinical services and professional expertise to patients on the Transition Care Program as part of the Integrated Care Rehabilitation Service.
The position will focus on providing Social Work interventions for the rehabilitation of older people who have been discharged after an acute or sub-acute episode of care; preventing inappropriate and avoidable admissions to acute care or permanent residential aged care; and supervision of Allied Health Assistants in their implementation of delegated therapy programs.
