We are seeking interest in the position of Key Worker at Community Living Program.
Community Living Program works alongside people with intellectual and cognitive disabilities to build their capacity to work towards their hopes, dreams and goals.
We work alongside individuals and significant people in their lives inclusive of families, friends and other formal supports. We provide holistic individual casework; facilitate social and capacity building groups; and engage in projects and community work.
We value relationship-based, trauma-informed, capacity building developmental and disability informed practice.

Questions
Q1: Outline your experience working alongside people with an intellectual or cognitive disability?
Q2: Outline your experience in conducting social work assessment, planning interventions and monitoring and evaluating support provided using person-centred and strengths-based approaches.
Q3: How do you operate as a member of a team, including any teams and stakeholder groups you are a member of?
Q4: Please read the attached CLP Practice Framework and outline how you might approach supporting people to build resources, relationships, knowledge and decision-making. (please use practice examples where relevant)
Q5: Please confirm you have a social work qualification and an AASW membership, a current Blue Card, a NDIS Worker Screening Card and a driver’s license and access to a personal vehicle for occasional work purposes.
