Brisbane South PHN is seeking a full-time Social Worker to be based in Redlands for a pilot project until 30 June 2026. This role is best suited to an experienced (5 years of experience) and skilled professional with a robust professional social work identity and a strong background in complex systems and multidisciplinary teams. It is not suitable for new graduates or those with limited experience.
The project requires a generalist social worker. While accreditation as a mental health social worker is welcome, a singular focus on mental health counselling falls outside the scope of this role.
This position is part of a pilot project embedding social workers into general practice. Three demonstration models are being trialled, each co-designed with participating stakeholders to meet the needs of specific population cohorts. This role will be based in the Redlands region to support access to social worker services for people residing in the Southern Moreton Bay Islands who have disproportionate access to care compared to the rest of the population, resulting in poorer health outcomes.

The successful applicant will be expected to contribute to:
Person-centred care:
Deliver person-centred care that improves health outcomes by helping the practice team understand and respond to the broader biopsychosocial needs of patients, and by connecting patients to relevant local support services. Development of professional networks with external service providers, carers and support networks of patients, and agencies.
Team-based care:
Work as a valued member of the interdisciplinary general practice team, complementing the scope of other professionals. Establish referral and communication pathways within the practice.
Health outcomes:
Address psychosocial issues and social determinants of health that impact patient outcomes, helping patients access existing supports and work toward short-term goals when service gaps exist. Effective use of outcome measurement tools and key performance indicators linked to patient outcomes, such as PROMIS 10 Global.
Social health model:
Navigate the challenges of general practice settings and effectively embed your role to deliver care aligned with a social health model.
System navigation and care coordination:
Coordinate care for high-risk, high-need populations, including leading case conferences involving GPs and nursing staff, in line with MBS requirements.
Use of MBS items:
Quickly understand and apply relevant MBS items that support interprofessional care involving both social workers and GPs.
