Location: Southport

Remuneration: $128 169 - $137 918 per annum

The new SMHRU unit offers mental health rehabilitation services for individuals needing extended inpatient care in a secure environment. Our focus is on autonomy, self-determination, and working collaboratively towards independent living. All consumers are over 18 years of age.

About Your New Role

Are you looking for a role with real meaning and purpose? Gold Coast Health have an exciting opportunity for an experienced NDIS and Length of Stay Discharge Coordinator to join our newly developed Secure Mental Health Unit and make a positive difference in the lives of others.

Key Responsibilities

  • Deliver high level clinical Mental Health services in accordance with discipline specific professional standards, code of ethics and legislation
  • Provide high level independent mental health and complex systems knowledge and skill, inclusive of assessment, intervention, report writing and discharge planning for complex discharge pathways
  • Provide high level guidance, leadership and advice for clinical and systemic escalations regarding complex length of stay matters
  • Develop and lead innovation, quality activities and systems improvements to build capacity within the Mental Health service for improved clinical practice, outcomes and continuity of care for consumers with complex discharge needs

 Qualifications

To be successful in this position you will require:

  • Tertiary degree in Occupational Therapy, Psychology or Social Work
  • A graduate certificate or greater within a relevant specialty area is highly desirable
  • Registration with AHPRA or eligibility for registration with AASW
  • Driver licence
  • NDIS experience is highly desirable
  • Blue Card/Working with children check (GCH will cover cost)
  • Evidence of vaccination, or be able to be vaccinated against and remain vaccinated against - Measles, Mumps, Rubella, Varicella, Pertussis and Hepatitis B
  • Tuberculosis screening, to identify if the tuberculosis vaccination may be required
  • Declaration of serious discipline history must be disclosed
  • Criminal History Screening will be required for new employees (GCH will cover cost)
