About the Role
The Kingston Centre Social Work Department is looking for an experienced Grade 1 social worker to join the team working in the sub-acute setting. This position is a fixed term part-time position and is part of a flexible team who collaborate to provide a holistic approach to service delivery at the Kingston Centre. This clinical role provides an opportunity to showcase your skills in working with people from a range of diverse backgrounds as well completing complex psychosocial assessments and development of patient centred care plans. Sub-acute services offers specialty inpatient rehabilitation and geriatric evaluation and management, focusing on older people who have reduced functional independence.
You will be a member of a dynamic Social Work and Allied Health team working in a collaborative model of care across Monash Health. The position is located at the Kingston Centre and works closely with the internal and external stakeholders to provide care founded on current evidence-based practice for patients accessing care through the inpatient and community based setting.
