East Grampians Health Service is seeking applications from suitably qualified and experienced Social Workers, with a commitment to excellence, to help drive improvements to the health and wellbeing of our community.

You will be part of a multi-disciplinary team, working in client homes, the community health centre, residential aged care and across our Ararat and Willaura campuses. You will provide timely responses to referrals, prioritising need, and applying social work theory to assessment and counselling interventions.
