About Social Work Services
The Social Work team across Monash Health is responsible for the delivery of a broad range of Social Work services for clients across the lifespan in the inpatient and outpatient setting.
About the Role
The Social Work Department is looking for an experienced Grade 1 social worker to join the team based on the Stroke and Neurosciences Unit at Monash Medical Centre. This is a full-time, permanent role. This role would involve working with patients who are suffering from neurological conditions or experiencing new or acute neurological changes. You will be working with patients from a diverse adult age group and offer support to patients and families around adjustment and complex social issues impacting on their recovery. This role is predominately based at Monash Medical Centre but work across all sites may be required. Hospital social work experience is desirable.
This clinical role provides an opportunity to showcase your skills in working with people from a range of diverse backgrounds as well as completing complex psycho-social assessments and development of patient centred care plans. It is a requirement of this role to participate in the Emergency Department and on-call rosters.
You will be a dynamic, innovative and flexible clinical practitioner with a good understanding of the treatment and psycho-social issues facing individuals with chronic illness and their families. You will also have an understanding of the aged care sector, chronic health conditions and related services and referral pathways. You will be able to demonstrate skills in psycho-social assessments, problem solving, communication and hospital discharge planning. You ensure that high quality Social Work services that place the client at the centre of are provided. Education, quality improvement and evidence based practices will be your key drivers. Due to your proven clinical experience, you will bring knowledge of healthcare reforms and initiatives and challenges that public healthcare is facing now and in the future.
