We are offering full time and part time positions for experienced, confident and energetic Social Workers to work in an acute hospital setting.

 

This role is aimed to improve the clinical care and management of older people who present to emergency departments and short stay non-geriatric wards. This is to reduce avoidable admissions of older people to hospital by providing expert comprehensive assessment, early identification of and identifying appropriate strategies to maintain maximum function and improved health outcomes.

 

Your role may involve aged care assessment, discharge planning and case management in a new model of care.

 

Nepean Hospital

• 1 FTE Social Worker 
• 0.5 FTE Social Worker 

Social Workers suited to this role hold tertiary qualifications in Social Work and having 5 or more years of postgrad experience with 3-5 years recent experience working in acute aged care. Your advanced clinical and problem solving skills and a passion for aged care will be demonstrated on a daily basis along with your ability to work autonomously and as part of a team.

• Start date: ASAP
• End date: July 2010.
